About Asarfi Hospital: business, products and customers

Asarfi Hospital runs a large multi specialty hospital and a separate cancer hospital in Dhanbad, Jharkhand, treating patients from the town and the districts around it. Its shares trade on BSE SME.

Asarfi Hospital began in Dhanbad in 2005 and now runs two hospitals there: a super specialty hospital with more than twenty departments led by cardiology, general medicine and neurosciences, and Asarfi Cancer Institute, opened in 2024 with money from its public issue, which offers medical, surgical and radiation oncology and nuclear medicine. The company says the cancer institute is one of only three cancer hospitals in Jharkhand. Revenue comes from inpatient stays, surgeries, outpatient visits and the medicines and consumables used in treatment.

A hospital's doctors, nurses, equipment and building cost much the same whether beds are full or empty, so profit depends on how many beds are filled and on the case mix: complex cardiac, neuro and cancer work earns far more per bed day than general medicine. New beds first pull occupancy and average revenue per bed down, as the company says happened when it added twenty critical care beds. About half of revenue comes from insurers' administrators, government departments, corporate tie ups and state schemes rather than cash patients, and these payers set package rates and pay months later, so money owed to the hospital is large against its sales.

Read more about the business

The offer document compares Asarfi with three listed hospital groups: Shalby, Global Health and KMC Speciality Hospitals, all multi specialty hospital operators, the last of them a regional one. What is new: an upgraded cath lab, a nuclear imaging camera at the cancer institute, a planned bone marrow transplant unit, a multi organ transplant tie up with Gleneagles Hospital, Chennai, and an education arm building a healthcare management institute in Ranchi.

From the offer document (prospectus dated 21 July 2023), the annual report for FY26, the May and August 2026 investor presentations, the May 2026 earnings call and the CARE rating note of September 2025.

What the company said, and what happened

The company's own statements, attributed, set against what it later reported; not newboard's estimates

The company says it aims for about ₹260 cr of revenue in FY27 and about ₹400 cr in FY28, with more than five hundred beds and an EBITDA margin of 23% to 25% under its Vision 2028; the CFO said the FY27 aim includes both new beds and an acquisition. It plans to take the cancer hospital from 65 to 150 beds by the end of FY27, add beds at the super specialty hospital if demand allows, start a bone marrow transplant unit once the state approves it, launch courses at its Ranchi institute by FY28 and move from BSE SME to the main board.

Is it a good business? Six questions

Each answer is about that one question, read from the documents; tap one for the evidence. Not a view on the shares.

Asarfi Hospital earns money by filling beds in a region with few large hospitals, and its cancer hospital faces very little nearby competition.

Its weak spots are slow payment from government schemes and insurers, and a single town that carries all its revenue.

AverageCan it set its own prices?Cash patients and complex cases give it room on price, but about half its revenue comes at rates set by schemes and insurers.
The evidence. The CFO says patients from distant districts are ready to pay more, while Ayushman Bharat has very small package rates and the company favours schemes with better rates.Earnings call, 27 May 2026, p.6, p.7 and p.9
StrongHow strong is it against rivals? It is the only super specialty hospital of its kind in the Dhanbad region and one of very few cancer hospitals in the state.
The evidence. The company calls its super specialty hospital the first and only one in the Dhanbad region and its cancer institute one of three cancer hospitals in Jharkhand; the CFO says the state has fewer than five hundred operational cancer beds.Investor presentation, August 2026, p.18; earnings call, 27 May 2026, p.13
AverageWho are its customers?Patients are many and small, but about half the money comes through a handful of payers such as state schemes, railways, Coal India and insurers.
The evidence. CARE says about 50% of revenue comes from TPAs, government departments and corporates; a change in a Jharkhand scheme's approval process cut cancer patients in the March 2026 quarter.CARE rating note, 26 September 2025, p.1; earnings call, 27 May 2026, p.8
WatchDoes it need a lot of money?Hospitals need buildings and equipment, and slow paying schemes tie up cash on top.
The evidence. The CFO puts receivables ageing at six to seven months and said capex this year would be under ₹15 cr with ₹8 cr to ₹10 cr in the bank, so some borrowing is likely.Earnings call, 27 May 2026, p.13
StrongCan it keep finding new work?Patient numbers keep rising and the company is reaching into neighbouring districts, with spare beds already built.
The evidence. Inpatients crossed sixteen thousand and outpatients about one point three eight lakh in FY26; the company says it has expanded into Godda, Sahibganj and Pakur.Earnings call, 27 May 2026, p.4; investor presentation, August 2026, p.16
AverageDo good and bad years swing a lot?Demand for treatment is steady, but scheme approvals and payments can move a quarter quickly.
The evidence. Cancer hospital surgical and medical oncology revenue fell in the March 2026 quarter during a pause in Ayushman Bharat cases, and the CFO links scheme payments to political stability.Annual report FY26, p.28; earnings call, 27 May 2026, p.8
